8D, the eight disciplines, is a structured method for solving quality problems, especially complaints from customers. It was developed at Ford in the 1980s and is standard in the automotive industry, where customers often require suppliers to answer a complaint with an 8D report. The method makes sure a team does not stop at a quick fix but finds and removes the root cause.

The Eight Disciplines

  1. D1 Team: form a team with the right knowledge.
  2. D2 Problem description: describe the problem precisely, for example with is/is-not analysis: what, where, when and how much.
  3. D3 Containment: protect the customer immediately, for example by sorting stock or adding inspections.
  4. D4 Root cause: find why the problem occurred and why it was not detected, with tools such as the 5 whys and Ishikawa diagrams.
  5. D5 Corrective actions: choose permanent actions that remove the root cause.
  6. D6 Implementation and validation: implement the actions and prove they work.
  7. D7 Prevention: change processes, standards and similar products so the problem cannot recur elsewhere.
  8. D8 Closure: close the case and recognise the team's work.

Many companies add a D0 for planning and initial emergency actions.

8D and CAPA

8D is a specific way to carry out CAPA: both lead from a problem to a verified fix and prevention. 8D adds a fixed structure and a standard report format that customers recognise, which is why it is common in supply chains with demanding customers.

8D in Software

8D reports in Word or Excel are hard to track across many complaints. Software guides the team through each discipline, records evidence, assigns actions with deadlines, links the case to products, batches and suppliers, and produces the report in the format the customer expects.
